I came within 6 inches of killing a volunteer this weekend.  I assume a 42lb weight for distance slammed into your face at 10 feet would be pretty life threatening.  I under-rotated on the second turn and threw it down the right sector line (if there were sector lines)  It flew straight over her shoulder, brushing her hair out of the way.  I watched the whole implement flight.  The AD gave very complete directions during the pre-event meeting and everyone in the area had been to many events before...and yet....

What things have you done at games to improve safety...and what more can be done?

The number 1 thing is personnel placement. If I understand correctly, if you're a right-handed thrower, she was on the right side of the trig, i.e., in the kill zone. Nobody should be there at all, because that's where a really dangerous miss is likely to go. If I'm judging, the throw doesn't happen until that side is completely clear, and we switch it for lefties. It's much better if everyone is behind the back line of the box. It's pretty unlikely that a weight will leave the back of the box with a lot of energy and a high arc, but even there vigilance and distance help.
